QUALIFICATIONS:

  • Highschool Diploma/GED required; Associates/Bachelors in relevant field preferred.
  • 1-3 years’ experience in Administrative, Healthcare or Community Health setting.
  • Excellent customer service skills to interact with clients, families and internal/external stakeholders.
  • Strong communication and engagement skills.
  • Strong organizational and time management abilities.
  • Ability to interact in a positive manner with families and children.
  • Excellent computer skills including knowledge of Microsoft Office.
  • Must have a valid driver’s license, and proof of current auto insurance with no more than 5 points on your MVR.
  • Must be able to pass a criminal background check.

  • Process referrals received by program.
  • Prioritize referrals to promptly get care for clients.
  • Cultivate and manage referral partner relationships allowing for streamlined communication.
  • Support clients throughout the referral process.
  • Provide ongoing communication and support to ensure successful outcomes.
  • Collaborate with internal teams and external stakeholders.
  • Optimize and grow client referrals.
  • Provide follow-up support to ensure successful referrals.
  • Identify new potential partners based on referral trends and patterns.
