Job Details Job Location: Tucson, AZ Position Type: Full Time Salary Range: Undisclosed Description Please Note: This is a grant funded position through January 5, 2027 only. Purpose The Outreach and Transition Care Coordinator plays a vital role in supporting women as they navigate high-risk transitions on their recovery journey. This position focuses on Transitional Wrap-Around Support, ensuring that participants receive consistent, compassionate, and comprehensive assistance as they move through each phase of the treatment process and transition back into the community. The Outreach and Transition Care Coordinator also establishes and maintains relationships and partnerships to support the organization, with a particular focus on referral development through collaboration with healthcare partners, social and human service agencies, community centers, and other providers. In short, The Outreach and Transition Care Coordinator provides community outreach, care coordination, and other ongoing follow-up services to help members achieve stability and long-term recovery. Essential Functions Conducts outreach to individuals transitioning from incarceration, detox, and other referral sources into The Haven. Contacts individuals and organizations in need of education or resources on substance use and mental health treatment and facilitate educational meetings on The Haven’s services. Identifies opportunities that enhance the resources, reputation and relationships of the organization. Meet with current and prospective referral sources to identify barriers in the referral process, report the findings, and collaborate with the admissions team to implement improvements that increase referral conversions. Create a written "Book of Business" of local referral sources and build a rotating calendar to visit and check in with referral sources weekly. Conducts prescreening for individuals seeking admission to The Haven for treatment and coordination with Admission department as necessary. Coordinate with the Admissions Supervisor and their team to qualify and ensure a warm hand-off for new referrals. Provides ongoing support for members throughout their time at The Haven. Coordinates with employees across The Haven’s programs, including member treatment teams, to provide support and ensure seamless care coordination. Provides follow-up support for up to three to six months, as needed, after a member’s discharge from The Haven. Conducts post discharge surveys. Other Duties and Responsibilities Maintains records of all outreach, facilitation, education and coordination activities including members and organizations involved. Works with leadership to aggregate data as indicated. Enters data regarding pre-screenings into the electronic health record system and maintains all protected health information in accordance with federal laws and rules. Completes all tasks in a timely manner based on time frames established by the designated supervisor. Maintenance of professional boundaries with members and organizations in need of education and/or resources for treatment of substance use disorders. Maintains adequate records of all miles driven, along with other travel costs, in order to receive reimbursement. Represent the organization at regional and community events, meetings, and conferences as appropriate. Performs other duties as assigned by designated supervisor. Qualifications Qualifications High school diploma or GED required Must be certified as a Peer and Recovery Support Specialist Must be at least 21 years of age upon hire Must have an Arizona Level-1, Non-IVP Fingerprint Clearance Card upon hire and maintain it throughout employment. Must have a valid Driver’s License and be insurable under The Haven’s insurance policy Must be able to work evening and weekend hours as necessary Excellent communication and customer services skills required Must possess competent computer skills, including use of Microsoft Office Ability to communicate the organization’s vision and impact to a diverse group of constituents. Ability to present ideas effectively, in both oral and written form Ability to prepare detailed reports Ability to contribute to a collaborative and professional team. Ability to build relationships and collaborate in a complex environment with member, staff, referral agencies, community partners, and others. Previous outreach experience is preferred Pervious case management experience is preferred Previous experience in behavioral health is preferred
