Description Job Summary: The Outreach & Drop-In Case Managers engage youth experiencing homelessness through street outreach and drop-in services, building trust and connecting them with resources to support health, safety, and long-term stability. This role may serve in: Outreach: Engaging youth in the community, including public spaces and street environments, to build relationships and connect them with available services. Drop-In: Providing direct support to youth at The PLACE’s Drop-In Center, including basic needs assistance, resource navigation, case management, and group facilitation. This role focuses on meeting youth where they are and guiding them toward safe, stable housing and supportive services. Job Duties: The duties of this position are performed personally, in cooperation with your supervisor, and/or in coordination with other staff. Conduct outreach in community spaces, providing information, crisis intervention, and referrals. Support youth accessing Drop-In services by connecting them to housing, healthcare, education, and employment resources. Provide case management, including assessments, goal planning, and follow-up. Maintain accurate documentation in accordance with organizational and funder requirements. Build strong, trusting relationships with youth while promoting trauma-informed care and positive youth development. Collaborate with community partners to strengthen service coordination. Drive to outreach locations, client appointments, and community spaces to provide services and maintain engagement. Transport youth to appointments, services, and other required activities. **Please note that this job description is not a comprehensive list of all tasks or responsibilities required for this role. Duties may change at any time, with or without notice. ** Requirements Minimum Qualifications: Bachelor’s degree in human services or related field; OR two (2) to three (3) years relevant experience. At least one (1) year of experience working with youth or vulnerable populations preferred. Valid driver’s license, clean driving record, and reliable transportation required to be insurable under The PLACE’s auto policy. Demonstrated understanding of trauma-informed care, harm reduction, positive youth development, and housing-first principles. Strong interpersonal, communication, and organizational skills. Ability to work independently and as part of a multidisciplinary team. Proficient in maintaining professional boundaries and confidentiality. Ability to act with integrity, professionalism, and confidentiality. Excellent written and spoken communication skills. Excellent interpersonal, negotiation, and conflict resolution skills. Must be computer proficient and possess experience of Microsoft Office Suite (e.g. Outlook, Word, PowerPoint, and Excel. Work Environment: Employees in this position may be subject to long hours on their feet in an office, outdoor, and other environments that may frequently include physical conditions such as inclement weather or temperature extremes, sharps, drugs and paraphernalia, and interpersonal contact from non-employees that include sexual innuendo and raw, abusive or threatening language. Employees must be comfortable providing safer sex supplies and bleach kits to youth. The PLACE provides personal protective equipment (PPE) and safety training to prepare and protect The PLACE employees in these uncontrolled environments. Compliance with safety protocol is required, which may include the use of PPE, involvement of law enforcement, and/or enactment of emergency protocols, detailed in The PLACE’s Emergency Operations Plan. This role requires regular use of a personal vehicle to meet with clients at their residences, attend appointments, and transport clients as needed. Physical Requirements: Employees are frequently required to sit, stand, walk, use hands to handle or feel objects, tools, or controls, reach with hands and arms, and occasionally stoop, kneel, or crouch Requires manual and finger dexterity and eye-hand coordination Able to speak and write the English language in an understandable manner Requires the ability to use department equipment, such as telephones, personal computers, adding machines, copiers, fax machines, etc. Position requires frequent use of computer and phone Able to hear well enough to communicate with employees and others Able to be on feet and mobile for up to 8 hours a day Able to lift up-to 50 lbs.
