ALLAMBI CARE ARE CURRENTLY RECRUITING FOR THE POSITION OF FAMILY PRESERVATION CASEWORKER, WITHIN THE OUTREACH YOUTH HOPE PROGRAM BASED IN CHARLESTOWN.

MINIMUM REQUIREMENTS

  • 2 years working with children and young people in a similar environment to AC
  • Extensive experience in Microsoft Office Applications
  • Knowledge of Needs Based Restorative Framework concepts and strategies
  • Tertiary qualification in related discipline (preferably Social Work)
Responsibilities
  • Collaborate with the Manager on the intake and induction of new families
  • Follow Allambi risk assessment procedure when visiting and meeting families for the first time, and ongoing
  • Have regular case planning meetings with the family and young person ensuring to use a collaborative approach to any goals.
  • Review and assess progress of case plan goals regularly with the Manager and discuss any barriers to their completion, in conjunction with monthly review meetings with the family
  • Work with families’ intensely in the initial period of their referral and develop a step down process as their needs reduce.
  • Ensure an exit planning meeting is held a month out from the referral ending and then fortnightly until the placement comes to a close, to ensure that all goals have been effectively meet and that the family is well supported and comfortable when supports are removed.
  • Maintain daily case notes of all visits/contact/meetings with families and others
  • Develop appropriate safety plans relevant to CS referrals, SDM outcomes and any new ROSH notifications
  • Work flexible hours including weekends and evenings were needed to support the family and young person, and maintain contact regularly throughout the week.
  • Escort clients to appointments and other important commitments were necessary, as well as organize youth work support and respite periods were needed. This is done in conjunction with the Manager and as outlined in the case plan for the family
  • Engage with Clientele and families to build a positive connection
  • Ensure that all documentation is up to date in accordance with Allambi standards including case notes, incident reports and case plans
  • Supervise and support youth workers in their role with the family and young person
  • Ensure the Manager is informed of any critical incidents especially those that may involve debriefing for workers
  • Communicate and work with Community Services as outlined in the Family Planning agreement
  • Work with clinical services internally and externally when meeting the needs of the family and young person
  • Attend ongoing internal and external training.
  • Promote AC Needs Based Restorative Framework culture and work to NBRF ideals
  • Collaborate with other NGO’s and government departments and maintain the relationships set up with external services
  • Work in the perimeters of the allocated budgets
  • Make referrals to programs that will gain a positive outcome for the young person and their families
  • Assist the family and young person to gain practical life skills and independently access supports in the community
  • Collaborate with the OCG and the Office of the Ombudsman as needed
  • Meet weekly with the program manager
  • Attend fortnightly team meetings with youth workers and the family where suitable
  • Ensure client files are maintained, accurate, up-to-date and to the required standards
  • Ensure that in all cases you act in a professional and appropriate manner at all times
  • Attend court and caution meetings with clients and parents as needed
  • Liaise with appropriate services to develop inter-agency protocols
  • Maintain and ensure registration and services are conducted at the pre-determined intervals for contracted company vehicles
  • Advise the Manager and Human Resources of any grievances, issues or concerns regarding employees and family’s
  • Attend relevant training sessions
  • Negotiate and achieve positive outcomes when problem solving
  • Prioritise, organise and complete set tasks given according to demanding time lines
  • Work both independently and part of a team
  • Any other tasks assigned by the Department Director
