We’re now looking for a passionate and driven Outreach Manager to lead our growing outreach service across The Wirral and Liverpool
Outreach support means delivering flexible, person-centred care for people in their own homes or out in the community — helping individuals live as independently as possible while being safely supported with the things that matter most to them. You’ll be:

  • Leading, motivating, and supporting outreach teams to deliver consistent, high-quality support
  • Managing new referrals, assessment and transition processes with local partners
  • Overseeing safe staffing, rota planning, and service delivery across multiple outreach packages
  • Ensuring compliance with CQC regulations and safeguarding requirements
  • Building strong relationships with families, commissioners, health professionals, and internal teams
  • Monitoring outcomes and quality assurance, ensuring person-led support that meets individual goals
  • Contributing to service development, growth planning, and business targets
  • Creating a culture that reflects our TEAMS values:
  • Transparency, Engagement, Authenticity, Motivation, Sustainability
  • Working closely alongside and being supported by our Registered Manager, who provides day-to-day oversight, guidance, and additional leadership capacity

Responsibilities

WHAT YOU’LL BE DOING

This is a leadership role where you’ll be both strategic and hands-on. You’ll be responsible for managing outreach packages that support people with learning disabilities, autism, and complex needs to live independently in their own homes and communities.

You’ll be:
