Job Description Contacts and schedules patient/client’s appointments for consultations, meetings with community agencies and/or diagnostic tests. Sets-up and maintains files and records. Collects and organizes statistical reports. The Outreach Services Associate is responsible for delivering community-based health services, including rapid testing for HIV, syphilis, and hepatitis (Hep B & C). This role involves engaging diverse populations in community settings, providing education, conducting screenings, and linking individuals to care and support services.   Job Responsibility * Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests. * Collects and organizes data required for statistical reports and completes reports, as required. * Attends and participates in meetings as appropriate. * Sets-up files and maintains records. * Provides incoming and in-process orientation. * Based on position, may be required to provide bi-lingual translation and/or guidance.   * Conduct rapid testing for HIV, syphilis, and hepatitis B/C in community settings (e.g., mobile units, shelters, events, street outreach).  * Provide pre-test and post-test counseling in a culturally sensitive and non-judgmental manner.  * Educate clients on prevention strategies, including safer sex practices, PrEP/PEP awareness, and harm reduction.  * Accurately perform fingerstick blood collection and handle specimens per protocol. * Maintain proper documentation of test results and client interactions in compliance with confidentiality laws (e.g., HIPAA).  * Facilitate referrals and linkage to care for clients who test positive or require additional services.  * Distribute prevention materials (e.g., condoms, educational resources).  * Participate in outreach events and collaborate with community partners.  * Maintain inventory of testing supplies and ensure proper storage and transport. * Follow all safety, infection control, and quality assurance protocols. * Enter data into EMR and other data collection system. * Performs related duties as required. Duties not mentioned here, but considered related are not essential functions.   Job Qualification * High School Diploma or equivalent required. * 0-1 years of relevant experience, required. * Previous experience in HIV / HIV prevention case management, preferred.  * Ability to communicate effectively while being sensitive to cultural diversity. * Prior experience with the LGBTQ community, preferred * Ability to work evenings and weekends, as needed.  *Additional Salary Detail  The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
