Outreach Specialist – Home4Good (Outreach Innovation) Location: Delaware - Statewide Schedule: Full-time | Community-based | May include evenings and weekends Program: Home4Good About Impact Life Impact Life is a mission-driven nonprofit organization dedicated to improving the health, stability, and well-being of individuals and communities through harm reduction, recovery-oriented, and whole-person care approaches. We meet people where they are and walk alongside them as they navigate systems, housing, and healing. About the Home4Good Program Home4Good is an outreach-focused, peer-led program designed to engage individuals experiencing unsheltered homelessness. Through consistent street-based outreach, trust-building, and strong partnerships, Home4Good connects people to housing, healthcare, and supportive services while reducing barriers to access. Position Summary The Outreach Specialist – Home4Good is a community-based, street outreach position focused on engaging individuals experiencing homelessness in unsheltered settings. This role emphasizes relationship-building, harm reduction, and connection to housing and services. The Outreach Specialist works closely with the Home4Good Case Manager to support housing navigation through warm handoffs, coordination, and follow-up. This position does not provide long-term or clinical case management. This role is about presence, persistence, and meaningful engagement. What You’ll Do Conduct consistent, street-based outreach to individuals experiencing unsheltered homelessness in encampments, public spaces, and other community settings Build trust and rapport through trauma-informed, person-centered, and harm-reduction-focused engagement Use lived experience, when applicable, to provide peer support, encouragement, and system navigation Identify immediate needs related to housing, healthcare, documentation, income, food, and safety Collaborate closely with the Home4Good Case Manager to support housing navigation and placement Provide warm handoffs to internal Impact Life programs and external community partners Accompany participants to appointments or service connections when appropriate Advocate for participants while maintaining professional boundaries Accurately document outreach activities, engagement efforts, referrals, and outcomes Participate in team meetings, supervision, and program coordination activities Follow all Impact Life policies, confidentiality requirements, and safety protocols What We’re Looking For At least one year of experience in outreach, housing services, social services, behavioral health, or a related field Experience working with individuals experiencing homelessness or other vulnerable populations strongly preferred Lived experience related to homelessness, substance use recovery, or system involvement is highly valued Strong communication and relationship-building skills Ability to work independently in community-based and nontraditional settings Comfort working in outdoor environments and dynamic situations Commitment to harm reduction, trauma-informed care, and dignity-centered engagement Work Environment Primarily field-based, including outdoor and street outreach settings Schedule may include evenings or weekends based on outreach needs Requires walking, standing, and travel throughout the community Why Join Impact Life Be part of an innovative outreach program making real, measurable impact Work alongside passionate peers and professionals committed to equity and access Support individuals on their path to housing and stability Join an organization that values lived experience, collaboration, and compassion Health Care Plan - Medical, Dental & Vision Retirement Plan - 401k & ROTH 401k Paid Time Off 11 Paid Holidays Yearly Training & Development Wellness Resources
