Job Details Job Location: San Jose, CA 95122 Salary Range: $36.92 - $41.85 Hourly The PACE Physical Therapy Assistant's responsibilities are to assist the PACE Physical Therapists provide treatment or maintenance that improves or maintains participant mobility, relieve pain, and prevent or lessen the physical disabilities of the participants. ESSENTIAL JOB FUNCTIONS: Assists or supervises exercises and therapeutic procedures (per request of therapist). Delivers physical modalities (PAMS) as instructed by the therapist. Sets up equipment for treatment and maintains a clean, well-organized rehabilitation gym/wellness center. Observes participants during treatment and maintenance to compile data on participants' responses and progress and provides updates to PACE Center Manager and PACE Rehabilitation Team Lead. Instructs participant and their decision-makers regarding in-home exercise program, bed mobility, gait training, transfers and on the care and use of assistive devices. Motivates, safeguards, and assists participants in exercise programs and functional training. Remains alert to pertinent input from other team members, Participants, and caregivers, and updates the IDT promptly regarding any changes in Participant’s condition or medical status. Provides training to professional and paraprofessional staff in body mechanics, use of equipment, bed mobility, transfers, gait patterns, home exercise program, etc. Maintains productivity per program expectation, necessary record keeping functions, and confidentiality pertaining to participant records, reports, and discussions. Provides home visits and home health as needed. Utilizes Epic appropriately and efficiently and serves as content and workflow expert for resolving issues and proposing solutions in Epic. Knows and adheres to the philosophy and goals of NEMS PACE. Complies with federal and state regulations. Complies with APTA code of ethics and continuing education requirements. Performs other job duties as required by Manager/Supervisor. Qualifications QUALIFICATIONS: Required: Licensure or Certification as required by State guidelines and California Board of Physical Therapy. Current BLS and First Aid required. Preferred: Minimum one year of experience working with a frail and/or older adult population At least one year of clinical experience working with the older adult population. One year of professional experience in Physical Therapy in a health care setting (not including internship). Possessing basic knowledge in all the modalities. LANGUAGE: Must be able to fluently speak, read and write English. Cantonese and/or Mandarin are preferred. Fluency in other languages in an asset. STATUS: This is an FLSA Non-exempt position. This is an OSHA high-risk position.
