JOB OVERVIEW

The PACE Social Worker - MSW plays a crucial role in providing comprehensive support to participants within the Program of All-Inclusive Care for the Elderly (PACE). This position involves working collaboratively with a interdisciplinary team to assess, plan, and implement individualized care plans that address the physical, emotional, and social needs of elderly participants. The ideal candidate will demonstrate a strong commitment to enhancing the quality of life for seniors while ensuring their safety and well-being.

EXPERIENCE

  • A Master’s degree in Social Work (MSW) from an accredited institution is required.
  • Current state licensure as a Social Worker is preferred.
  • Experience working with elderly populations or in geriatric care settings is highly desirable.
  • Strong interpersonal skills with the ability to communicate effectively with diverse populations.
  • Knowledge of community resources available for seniors and their families.
  • Proficiency in case management practices and documentation standards.
  • Ability to work collaboratively within a multidisciplinary team environment.
    This position requires a compassionate individual who is dedicated to improving the lives of elderly participants through effective social work practices.

    Pay: $42.00 - $49.00 per hour
