As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. Trinity is accredited by DNV Healthcare Inc. and has achieved Pathway to Excellence designation by the American Nurses Credentialing Center. General Established team of 1 Medical Director and 4 Advanced Practitioners Established practice serving the community Experienced staff, both clinical and non-clinical, who know the patients and the community Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination Responsibilities Average of 8-10 patients per day; no nights or weekends Shared call amongst the providers. Currently 1:17 – Triaged through MyNurse Assume PCP role while rounding in facilities to meet nursing home requirements and acute needs of patients with a focus on patient driven goals of care and quality of life Clinic hours: Monday-Friday 8am-5pm, no nights, weekends, or holidays, 4 day work week option available. Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care Patient-focused, friendly, and willing to collaborate in a team environment Excellent communication (written and verbal), analytical and problem-solving skills Qualifications Nurse Practitioner or Physician Assistant Experience in post-acute and/or long term care facility preferred Palliative Care certification preferred Ability to work as a team member Hold an unrestricted medical license in the state of Iowa Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice We offer Salary based position with performance incentive Attractive incentive package A competitive Total Rewards program offers benefits that align with your needs and priorities, no matter what stage of life you are in. Responsibilities
