Act as aclinical expert in palliative care, providing specialist advice, guidance, and supervision to clinical staff, patients, and carers. Promote best practice in palliative and end-of-life care through expert consultation and case management. Support the development of individualised care plans, ensuring they align with patient preferences and national palliative care standards. Work autonomously, managing a caseload of patients with complex palliative care needs while adopting the key worker role, as agreed with the Palliative Care Team Leader.
