Description

Providence Care House Calls is currently hiring a full time Nurse Practitioner to join our amazing team in the ( ) and surrounding areas of South Carolina to care for patients in their homes and facilities who are enrolled in our Palliative Care House Calls Program.

QUALIFICATIONS

  • Valid ANP license in the state of SC
  • 1 year of ANP experience preferred
  • Previous hospice, home health or palliative care experience a plus but not required
  • Passion for helping others
  • Willingness to travel to patients homes within the coverage area

Our practice managers and collaborating physicians are always available for consultation, mentorship and support. In addition, our electronic medical records (EMR) system supports our use of evidence-based practice.
    Job Type: Full-time
    Pay: $91,815.96 - $110,574.05 per year

Responsibilities
  • Offer skilled nursing / physician services to patients who suffer from advanced illnesses.
  • Conduct physical assessments, perform and interpret diagnostic tests, diagnose and treat common conditions, provide screenings, interventions, counseling, and prescribe medications as needed to patients in the geriatric population.
  • Provide wellness checkups, cognitive examinations and palliative care.
  • Refer patients to hospice care when/if warranted
  • Provide care in private homes and/or facilities
