Job Details Job Location: Stevens Point - Stevens Point, WI 54482 Position Type: Full Time or Part Time Salary Range: $48.07 - $64.90 Salary Job Shift: Day Palliative Nurse Practitioner – Part-Time & Full-Time Opportunities We are seeking a Nurse Practitioner to join our growing palliative care program. This is an opportunity to make a meaningful impact by supporting patients with complex, chronic, or serious illnesses in a collaborative, patient-centered environment. Position Details: Part-Time: 20 hours/week (non-benefited) Full-Time: 32–40 hours/week (benefited) Responsibilities: Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms Complete F2F encounters for hospice patients as needed Develop and implement individualized care plans in collaboration with the interdisciplinary team Prescribe medications and coordinate care for patients with advanced or chronic illnesses Accurately document all encounters in the EMR Communicate effectively with patients, families, and other healthcare professionals Requirements: Experience preferred but willing to train new Nurse Practitioners!
