Description:

Paramedic Practitioner/Nurse Practitioner The post-holder will be supported through indirect supervision to be responsible for assessing, and diagnosing, with progression to interpreting investigations, and treating patients within Primary Care, developing a scope of competence. They will monitor and assist with improving standards of care utilising clinical expertise and experience. They will teach and support colleagues where appropriate, providing professional clinical leadership. Where appropriate they will lead and/or participate in initiatives to improve standards of patient care.
The post-holder is service driven and will develop themselves using the Department of Health Guidance across the four domains of Advanced Practice, these are: 1. Expert Practice 2. Practice and service development, research and evaluation 3. Education and training 4.
Professional leadership and |Consultancy JOB PURPOSE The post holder will be an experienced Paramedic qualified to undergraduate level. They will: Be able to demonstrate safe, clinical decision-making by autonomously assessing patients presenting to Primary Care including initial history taking, clinical assessment and diagnosis. Following initial in-house training, they will be able to advise on treatment, and evaluate care with suggestions of (or give prescription if a prescribing qualification is held) any medication required. Demonstrate critical thinking in clinical decision-making.
They will work collaboratively with the practice team to meet the needs of patients and will play a key role in service delivery and adherence to policy and procedures. Provide and assure safe care to patients and maintain a safe environment for all according to local and national standards. Facilitate an effective workplace culture with the practice and in cross-organisation working if needed. Sustain person-centred, safe and effective care through self-awareness, leadership, active learning and development.
Constantly seek to further skills and knowledge to enhance patient care, meet the changing needs of the practice. 3. KEY RESULT AREAS/MAIN DUTIES AND RESPONSIBILITIES: 3.1.Clinical Practice 1. Work autonomously (within defined patient pathways of care) to assess, examine, investigate, diagnose and treat patients with a wide range of physical, social, and psychological needs and presenting complaints.
2. Undertake consultations in an appropriate format (face-to-face, telephone, virtual), taking into account the clinical presentation and patient preference. 3. Undertake telephone triage work and telephone advice calls.
4. Undertake home visits as assessed by the GP or Advanced Nurse practitioner. 5. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug treatments into a management plan.
6. Undertake appropriate investigations and treatments utilising evidence-based practice/clinical guidelines applying skill, knowledge and clinical judgement to meet the patient needs. 7. Interpret information using knowledge and judgement to provide appropriate advice when evaluating all aspects of the patient care and treatment plan, according to their holistic needs.
8. Discuss with registered GP if needed, and to make referrals to other professionals in primary and secondary care. 9. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
10. Support patients to adopt healthy lifestyles and apply principles of self-care. 11. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions (as appropriate).
12. Be aware of own limitations and refer to other practitioners when needs and risks are beyond their own scope of practice. 13. Obtain consent when undertaking assessments.
When discussing implementation of investigation and treatment plans, give alternatives, if appropriate to facilitate patient choice. 14. Evaluate the effectiveness of interventions and treatment using evidence-based practice, whilst using knowledge and skills to assess further patient needs. 15.
According to current legislation and practice policy undertake independent, non-medical prescribing if relevant. 16. Maintain accurate patient records, both written and electronic, in accordance with local and national professional policies and guidelines. 17.
Ensure that all practice within the clinical area complies with practice policies and procedures. 18. Practice in accordance with the professional and legal framework for advanced clinical practice and that of their registered professional body. 19.
Where issues of clinical competence are identified, work with their supervisor to address them. 20. Contribute to the resolution, investigation and follow action of any informal or formal complaints. 21.
To maintain a professional portfolio of evidence for personal & professional development. 22. Act as a clinical role model. 23.
Practice in a confident, competent professional manner. 24. Ensure dignity, privacy, cultural and religious beliefs are always respected. 25.
Be aware of statutory guidance for vulnerable and/or abused children or adults. 26.Work with the practice team and patients to ensure early recognition of domestic violence, vulnerable patients, substance abuse and addictive behaviour. 3.2.Practice and Service Development, Research and Evaluation 1. To participate in the systematic monitoring and evaluation within the clinical area.
Participate in the development of action plans to enhance quality using audit results. Participate in the evaluation of changes made in the practice. 2. Lead in setting and maintaining developments in evidence based practice.
3. Undertake data collection for recognised audits and service reviews. 4. To audit, evaluate and review the quality of their own work, and where necessary make appropriate improvements to raise standards and quality of performance.
5. To inform and influence Clinical Governance issues. 6. To ensure research, audit findings and recommendations are communicated to appropriate individuals.
7. Take an active role in evaluating current service provision and give constructive views on service development. 3.3.Educational Responsibilities 1. Facilitate the implementation of clinical competencies within the clinical environment.
2. Act as a mentor & preceptor to staff and students as needed. 3. Facilitate training and education within their area as well as in the wider arena as appropriate using specialist knowledge and skills to enhance the knowledge and practice of others.
3.4.Professional Leadership 1. Work collaboratively with secondary and community care teams, ensuring there is streamlined care across organisational boundaries. 2. Work with clinical and leadership teams to develop new and improved processes to improve patient flow and care processes.
3. To assist with development of new initiatives and ways of working, including nurse/AHP led services, detailed by local and national Policy. 4. To act as a positive role model to inspire those working within these services, to improve and develop services that promote the best interest of patients in primary care.
5. Participate in developing professional practice nationally through membership of relevant professional associations where appropriate. 6. Share best practice and new evidence through presentation at local meetings.
3.5.Communication 1. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. 2. Communicate with and support patients who are receiving bad news.
3. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, and preferred ways of communicating. 4. Anticipate barriers to communication and take action to improve communication.
5. Maintain effective communication within the practice environment and with external stakeholders. 6. Act as an advocate for patients and colleagues.
7. Ensure awareness of sources of support and guidance (eg PALS) and provide information in an acceptable format to all patients, recognising difficulties and referring if appropriate. 8. Make operational judgements, manage conflicting views and reconcile differences in a professional manner.
9. Develop and deliver presentations to clinical and academic meetings. 3.6.Managing information 1. Use technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting and communicating information.
2. Review and process data using accurate coding to ensure accurate information retrieval for monitoring and audit processes. 3. Manage information searches using the internet.
4. Understand responsibility of self and others at the practice regarding the Freedom of Information Act and patient confidentiality. 5. Collate, analyse, and present clinical data and information to the team.
3.7.Equality and Diversity 1. Identify patterns of discrimination and take action to overcome this, to promote diversity and equality of opportunity. 2. Enable others to promote equality and diversity in a non-discriminatory culture.
3. Support people who need assistance in exercising their rights. 4. Monitor and evaluate adherence to local chaperoning policies.
