Working autonomously but with full clinical support, you will manage patients presenting with a wide range of minor illness and injury, as well as long-term conditions where appropriate You will assess, diagnose, treat, refer or signpost patients who attend the surgery Provide clinical assessment, diagnosis, treatment, and evaluation of patients Work collaboratively with GPs, nurses, pharmacy Technician and admin team Support the management of long-term conditions and care planning Support and participate in shared learning across the practice Assess patients presenting with acute illness or for review of chronic disease conditions Undertake home visits as required Clinically examine and assess patient needs from a physiological and psychological perspective and plan care accordingly Advise patients on general healthcare and promote self-management where appropriate, including signposting to other community or voluntary services Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. Maintain accurate health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. Independent prescribing.
