As a Case Aide, you will play a vital role within our Housing Department, supporting residents as they work toward personal goals and long-term success. This position provides direct support to the Case Manager by helping ensure resident needs are identified, documented, and addressed in a timely and respectful manner. The Case Aide serves as a key point of support for residents, assisting with documentation, coordination of services, goal planning, and follow-up, while maintaining strict confidentiality and professionalism. Key Responsibilities The Case Aide duties include, but are not limited to: Ensure resident documentation—including discharge summaries, mental health evaluations, and medication records—is accurate, complete, and properly filed in resident folders Assist residents in developing individualized Goal Plans and participate in follow-up meetings to track progress Support coordination of services and resources needed by residents, in collaboration with the Case Manager and community partners Maintain confidentiality and appropriately manage sensitive and protected information at all times Assist with data collection and documentation for monthly and quarterly reports as requested by the Case Manager Provide general support to residents in a respectful, trauma-informed, and strengths-based manner Communicate effectively with residents, staff, and supervisors to support continuity of care Schedule 1 Part-time Case Aide: Tuesday, Wednesday, Thursday, Friday (20 Hours) 5:00 PM – 10:00 PM 1 Part-time Case Aide: Saturday, Sunday, Monday (15 Hours) 5:00 PM – 10:00 PM Salary $19.00 per hour Benefits Free YMCA membership Paid sick time My Better Benefits Employee Assistance Program Employee Assistance Program (EAP) Requirements Associate’s degree in Human Services, Social Work, or a related field preferred Must be at least 18 years of age Excellent verbal and written communication skills Knowledge of mental health and substance use disorders preferred Ability to maintain confidentiality and demonstrate professionalism at all times Strong organizational skills and attention to detail Dedication to helping others and supporting individuals through challenging life circumstances General knowledge of Microsoft Office (Word, Excel, Outlook) Experience collaborating with individuals with mental health diagnoses, physical health conditions, or substance use challenges preferred
