Aleria Home Care Agency LLC is seeking a compassionate and organized Part-Time Case Manager to join our team in Des Moines. This role is ideal for someone with healthcare experience who enjoys working directly with clients to ensure they receive the services and support they need. Key Responsibilities:

• Conduct comprehensive client assessments
• Develop individualized care plans
• Coordinate referrals to community resources and services
• Monitor client progress and update care plans as needed
• Maintain documentation in compliance with agency and HIPAA standards
• Collaborate with care providers, families, and agency team
• Attend team meetings and contribute to care discussions

Requirements:

• Prior experience in healthcare, case management, or social services
• Strong communication and time management skills
• Ability to manage a part-time caseload independently
• Documentation and organizational skills
• Reliable transportation and valid driver’s license (if travel is required)

Preferred Qualifications:

• Degree in Social Work, Nursing, or Human ServicesHome care or community-based case management experience
