YOUR CONTRIBUTION

The Diabetes Educator is responsible for the delivery, management and coordination of quality patient care
and service delivery.
The Diabetes Educator / Dietician within TAIHS uses professional knowledge and judgment to assess health
needs, provide patient centred culturally safe care and to advise and support people to manage their
health. They practice independently and in collaboration with other health professionals, perform general
clinical and administrative functions. They provide comprehensive assessments to develop, implement and
evaluate an integrated plan of health care, and provide interventions that require professional knowledge,
skills and clinical decision making. They do this in a range of settings in partnership with individuals,
families and communities.
This position of Diabetes Educator / Dietician will be based in Townsville working primarily from the
Garbutt clinic and at times will require working from Ingham and other outreach clinics.

Responsibilities
  • Providing diabetes education and dietary advice to patients
  • Contributing to multi-disciplinary case conferencing and management plans
  • Proactively recruit patients, facilitate GP referral and coordinate patient care
  • Undertaking all tasks within scope of practice and/or as directed
  • Achieve optimal MBS billing for all eligible services
  • Plan and delivery of quality diabetes care in consultation with the patient and the multidisciplinaryhealth care team
  • Work collaboratively with the TAIHS allied health and primary care team
  • Manage register of patients with diabetes or at risk of diabetes
  • Measure and report on diabetes clinical indicators including HbA1c, smoking status, BMI, renalfunction, chronic condition plan and health assessment attendance
  • Ensuring patient records are current and maintained in a confidential manner
  • Ensure patient clinical interventions are carried out in an effective manner
  • Manage procurement and supply of glucometers and continuous glucose monitoring equipment
  • Conduct opportunistic screening and follow-up of patients
  • Ensure patient allergy list is kept up to date
  • Maintain consulting rooms assigned, ensuring materials are fully stocked and tidied after eachconsult
  • Report any functional problems with clinic equipment in accordance with process
  • Comply with TAIHS Primary Health Care Procedures Manual in the delivery of care and services
  • Conduct full handover of patients to other health practitioners
  • Ensure infection control standards are in place in accordance with regulatory standards
  • Ensures progress notes, reports and records are completed in the required format
  • Accurately document in client files any treatment and medication information
  • Maintain recall and follow up of patients as per General Practitioner/s requests
  • Instruct client/care givers on quality use of medications
  • Instruct client/care givers on how to obtain new medication supplies when required
  • Pursues own professional development to fulfill registration requirements and actively engageswith company instigated training
  • Contribute to continuous quality improvement activities to achieve better outcomes for patientswith or at risk of diabetes
