Key Responsibilities: - Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team. - Provide symptom management, pain control, and emotional support to patients and their families in accordance with hospice care standards. - Prescribe medications and treatments within the scope of practice and ensure proper documentation of all clinical activities. - Educate patients and families about disease progression, treatment options, and end-of-life care preferences. - Collaborate with physicians, social workers, chaplains, and other healthcare professionals to coordinate comprehensive care. - Monitor patient progress and adjust care plans as needed to ensure comfort and dignity. - Maintain accurate and timely documentation in compliance with regulatory and organizational policies. - Participate in quality improvement initiatives and ongoing professional development. Skills and Qualifications: - Valid Advanced Registered Nurse Practitioner (ARNP) license in the state of practice. - Certification in hospice and palliative care preferred. - Proven experience in hospice, palliative, or geriatric care is highly desirable. - Strong clinical assessment and decision-making skills. - Excellent communication and interpersonal skills, with a compassionate approach to patient care. - Ability to work independently and as part of a multidisciplinary team. - Knowledge of hospice regulations, policies, and best practices. - Proficiency with electronic medical records (EMR) systems. Requirements REQUIREMENTS/QUALIFICATIONS: BSN and certification as an Advanced Practice Registered Nurse (APRN) from an approved Nursing program. Certification as a Nurse Practitioner (NP) with specialization in adult-gerontology, palliative care, or hospice care and a minimum of 2-3 years of clinical experience as a nurse practitioner, preferably. Currently licensed as a Nurse Practitioner in the state of Iowa. Two years of hospice or home health nursing experience preferred. Proof of driver’s license and current insurance coverage. Strong clinical assessment, diagnostic, and prescriptive skills. Knowledge of palliative care principles, pain management strategies, and end-of-life care best practices. Excellent communication, interpersonal, and counseling skills. Ability to work collaboratively as part of a multidisciplinary team and provide leadership in palliative care initiatives. Demonstrates leadership and decision making abilities through effective interpersonal skills, knowledge of current and future nursing trends, and adhering to ethics, standards, and the support of agency policies and procedures. Ability to maintain and update professional knowledge base and the ability to assimilate and utilize agency information regarding policy and procedure changes. Able to demonstrate competence in professional decision-making skills within the parameters of the scope of practice, including the ability to make appropriate assessments, problem-solve, prioritize, complete Plans of Care and evaluate their effectiveness. Able to use technology effectively and appropriately, including client care equipment, I-Pad and communication devices (smart phones, voicemail, and electronic mail). WORK ENVIRONMENT/EQUIPMENT: Employee will perform the duties in both inside and outside environmental conditions. Visits are conducted in patient homes requiring employee to drive from location to location during the work period within the service territory including Iowa City, small towns and rural areas. Atmospheric conditions – fumes, odors, dust, poor ventilation, or smoking. The ambient noise level in this environment can flex, is generally low. The duties include using office machinery including extensive use of computers, copiers, etc. The driving requirements include in-town and highway driving in good and inclement weather. All professional staff will use iPad to document work in client home and/or hospital settings. PHYSICAL/COGNITIVE DEMANDS: Performs skilled physical assessments and various skilled nursing activities, use of equipment for client care, computer and smart phone knowledge for documentation and communication that require fine motor coordination/dexterity, the ability to grasp, vision with or without corrective aids adequate for fine motor tasks, client assessment, and driving. Able to lift, push & pull 35 lbs often, occasional lifting greater than 50 lbs. Able to sit for long periods in a car. Requires the physical demands of climbing up/down stairs, walking, hearing, kneeling, standing, pushing, pulling, grasping, lifting, talking, hearing etc. with or without the use of assistive/adaptive devices. Requires visual acuity to operate motor vehicle, view electronic devises (I-phone, I-Pad, computer terminal) reading data and figures, visual inspection of patients. Cognitive ability to solve practical problems; variety of variables with limited standardization; interprets instructions.
