POSITION SUMMARY:
Participates as a member of the multidisciplinary team to plan, implement and evaluate programs, outings and events that meet the residents’ physical, social, emotional, cognitive, spiritual and cultural needs.
RESPONSIBILITIES
Documentation
· Complete initial and ongoing programs assessments as necessary to determine residents’ recreation and rehabilitation needs.
· Complete documentation and accurate records as required including Program guides, participation tracking, monthly statistics, Intake documentation, Quarterly documentation, MDS/CAPS, Care plan updates
Programming
· Plan and implement programs based on resident assessments completed
· Facilitate one-on-one and small group programs on unit
· Facilitate or Co-facilitate large group weekly programs including religious services and entertainment performances
· Plan and implement special events, community-based programs and outings
· Evaluate and make adaptations to programs as appropriate
Committees/Meetings
· Participate in department meetings, unit meetings, and multidisciplinary care conferences.
· Participate in Home’s committees as required.
· Facilitate Residents Council meetings as required (if applicable).
Communication
· Ensure effective communication and good human relations with residents, residents’ families, volunteers and staff
· Provide residents with a calendar of programs offered
· Ensures that posters, notices, and calendars are up to date, attractively displayed and easy for residents to read.
· Ensure open communication is provided to Manager of Programs and Community Development
Community Relations
· Develop community contacts and resources.
Professional Development
· Upgrade knowledge and skills relating to the position.
· Attend in-services as required.
· Develop research projects based on departmental mandate.
· Completes all assigned training required as per the LTCHA and Regulation 79/10.
Collective Agreement
· Have a thorough understanding of the current collective agreement
· Abide by the policies set out in the collective agreement
Quality/Risk Management
· Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
· Required to participate in the improvement of the indicators and achieve satisfactory results.
Other
· Physiotherapist assistance as required
· Complete other tasks as assigned by Programs Manager.
Physical Demands
· Periodically lifting may be required.
· Sitting at desk.
· Walking and standing throughout the shift
· Repetitive movement at or above the waist.
· Repetitive bending movement below the waist.
· Pushing movement may be required.
RESPONSIBILITIES TO HEALTH AND SAFETY:
· Is aware of employee’s responsibilities and follows all Health and Safety policies and procedures.
· Works safely to reduce the risk of injury to self, co-workers and residents
· Is alert to and properly reports all actual or potential hazardous situations to immediate supervisor.
· Is aware of WHMIS regulated products and takes safety precautions accordingly
· Does not operate or use faulty equipment
· Promptly reports personal injury to immediate supervisor and seeks first aid as needed
· Participates in fire safety demonstrations and fire drills and knows the Home’s emergency plan
· Maintains first aid box and renders first aid and CPR on outings if necessary.
· Is aware of and knows how to use personal protective equipment (PPE).
· Attend all mandatory health and safety training sessions
· Report hazards that you are aware of to your Supervisor and the JHSC
· Understand and follow all of the Home’s health and safety policies and procedures
