Medication administration; ordering and refilling medications, dispensing of all medications to residents and physician/ancillary treatments. Provide compassionate care and assistance to residents living with Alzheimer’s disease, dementia, or another memory impairment Act as liaison and maintain positive relations with residents, families, and physicians. Enhance the lives of older adults by assisting them with mobility needs, daily living activities, including, bathing, grooming, and routine personal tasks. Monitor for change of conditions. Develop comprehensive care plans based on each resident’s needs and other documentation as required. Collaboratively support the Program Coordinator with the supervision of care staff, volunteers, and other staff providing services to residents. Maintain current tasks lists for care staff, and update timely as ADL changes occur. Provides first aid and emergency assistance while working within the community. Monitors and records blood glucose readings for residents per the resident service plans. Completes incident reports and participates in corrective action plan development. Maintains electronic log with all vital information through Point Click Care. Follows all established Hollenbeck Palms Protocols for safety and infection prevention. Works as scheduled and demonstrates punctuality and dependability. Maintains accountability for understanding and following company and department policies and procedures as outlined in the Employee Handbook. Support other community departments when needed. Responsible for admissions, discharges, and transfers as necessary Other duties as assigned by the Director of Nursing Requirements Job Requirements and Qualifications: Applicants must possess a valid LVN Nursing license in the State of California. Dedication and passion to serve seniors with excellent customer service skills. A positive attitude, the flexibility to perform various duties in service to the residents, and the ability to work in a team environment are keys to success. Experience in long-term care community health, geriatric and/or rehabilitative aide, preferably with people with Alzheimer's/Dementia. Must be able to read, write, speak and understand English in order to assist in emergency situations. Must be able to understand, follow, support and initiate policies and procedures of the facility. Must be able to recognize emergency situations and respond appropriately. Must clear criminal background with DOJ and FBI Any experience with Electronic Documentation systems such as Point Click Care of similar- preferred. High School diploma/GED CPR/BLS-First Aid certification Must be at least 18 years of age
Responsibilities
