The Care Managers are responsible for coordinating care and services for patients. In an effort to meet recipients’ comprehensive health needs through communication, interventions, and available resources to promote quality cost effective care, CMs are to implement a collaborative process of: assessment, facilitation, education, care planning, evaluation, coordination of care and advocacy.   Care Managers are expected to work with the patients, utilizing a team approach, to develop individualized goals that lead toward enhanced self-management. Progress towards these goals must be assessed on an ongoing basis and revised as indicated. Candidate qualifications:  Associate’s degree or better from an accredited School of Nursing with an unrestricted license to practice nursing in NC or BSW/MSW from an accredited college or university. Minimum of (5) five years of nursing or SW experience in an ambulatory or acute care setting, home health, or public health. Prefer experience working with the behavioral health and substance abuse population. Required Skills: * Excellent verbal communication skills.  * Must be able to work with changing priorities.  * Requires excellent organizational, problem solving and critical thinking skills.  * Must be able to interact with individuals of all cultures and levels of authority.  * Requires the ability to maintain confidentiality.  * Must be able to function as part of a team. * Excellent and effective written and oral communication skills. * Proficiency in Microsoft Office Outlook, Word, Excel, PowerPoint use and e-mail communication. * Ability to communicate clearly and succinctly. * Dependable, manages time well; efficient and organized. * Ability to produce accurate work; ability to perform multiple tasks in a proficient and timely manner Additional skills preferred: Experience with electronic documentation systems.   Key Responsibilities 1. Proficiently and systematically uses data from Virtual Health, to target patients and providers for outreach, education, and intervention. 2. Assists and advocates for patients to access care, services, and treatments in the most cost-effective healthcare setting with emphasis on linkage to their medical home. 3. Addresses the total patient and support systems (i.e. caregiver, family, etc.), inclusive of medical, psychosocial, behavioral, cultural and spiritual needs. 4. Involves the patient and their support systems in the decision-making process. 5. Utilizes motivational interviewing techniques to measure a patient’s understanding and acceptance of the proposed plans, his/her willingness to change, and his/her support to maintain health behavior change. 6. Communicates and coordinates with all providers and members of the care team, in an effort to minimize fragmented care. 7. Documents clearly, completely and concisely in Virtual Health related to all case management activities. 8. Provides coaching, information and support to empower the patient to make ongoing independent and/or healthy lifestyle choices. 9. Monitors quality and effectiveness of interventions for the patient by setting specific, measurable goals. 10. Willingly performs other duties as assigned.   Kintegra Health Core Requirements 1.
