Job Description: Nurse Practitioner – Healthcare Partnership Program Job Title: Nurse Practitioner FLSA Exemption Status: Non-Exempt Organizational Alignment: Reports to your respective HPP coordinator Position Summary: As a Nurse Practitioner in the Healthcare Partnership Program (HPP), you will be placed within a partner organization to provide direct patient care to employees and their families. This part-time position offers a flexible schedule of 16 hours per week — Mondays 7:00 a.m.–3:00 p.m. and Wednesdays 11:00 a.m.–7:00 p.m. — with the potential to combine or increase hours in the future. This role focuses on primary, proactive and preventive care, managing chronic conditions, and addressing urgent health needs for the company’s employees, spouses, and dependents. You will work autonomously, collaborate with HealthBar’s broader healthcare team, and contribute to enhancing employee well-being through proactive health services. Essential Functions: Provide advanced clinical support to HPP and school patients, including acute and episodic care, primary preventative care, and chronic disease management. Collaborate with HPP nurses on claims analysis. Establish clinical pathways and treatment protocols for patients. Conduct telehealth visits for HPP patients. Coordinate care with the patient’s outside medical network. Act as a consultant in clinical settings, maintaining and promoting standards of care. Maintain required licensure and certifications for the role. Qualifications Required: Education: Graduation from an accredited nursing program (Master's degree in Nursing). Licensure: Active and unrestricted State of Michigan issued Registered Nurse License. Certification: Active and unrestricted FNP National Board Certification from one of the following organizations: American Academy of Nurse Practitioners American Nurses Credentialing Center Active and unrestricted State of Michigan-issued Registered Nurse and Nurse Practitioner Specialty Certification License. Experience: Minimum of 2 years providing care as a nurse in applicable areas such as med-surg, ER, ICU, acute care, public health, school nursing or outpatient offices (preferred). Basic Life Support (BLS) Certification from the American Heart Association or American Red Cross within 90 days of employment. Qualifications Desired: Department of Transportation (DOT) Examiner Certification (preferred; training provided if needed). Additional Certifications: Preferred and highly encouraged from an accredited certification board, before or after employment in a specialty field such as: Health Coaching Case Management Primary Care Hospital Based (Med-surg, ICU, ED based certifications) Women's or Men's Health Sports Medicine Physical Requirements: Ability to communicate verbally Fully functional vision is required (prescription lenses or contacts are acceptable) Prolonged periods of sitting, standing, or walking Ability to lift or carry up to 25 pounds occasionally Ability to operate a computer and other standard office or clinical tools Reliable transportation for occasional travel to partner locations Changing business conditions may trigger changes to physical requirements
