The PT RN will work 24 hours/week. Wage is $38,750 - $41,000 annually. This position will serve the areas of South St. Louis County and Jefferson County. Monitor the health and safety of the people we support. Complete face-to-face visits with the people we support as required to ensure health and identify concerns. Review and determine that care is being provided according to the Doctor’s orders and record accuracy. Conduct thorough health assessments to establish a well-coordinated healthcare plan to coincide with the person-centered plans. Ensure physician’s orders are present, current, and inclusive for all required information per policies and guidelines. Review medical records, including but not limited to, physician’s orders, Electronic Medication Administration Record’s (EMAR), and visit reports, on at a minimum monthly basis. Review and ensure accuracy EMAR’s as dictated by physician’s orders. Review employee entries in the EMAR system for accuracy. Communicate, train and follow up with concerns and discrepancies. Act as the medical expert to advocate for the people we support. Take immediate action to address urgent health concerns, conduct assessments when issues arise, implement appropriate interventions, and communication to the internal team and medical providers. Review medical records on an ongoing basis. Ensure medical equipment and supplies are available for use in homes. Conduct ongoing training, including but not limited to, personal care training, specialized diet and other nursing delegated training. Provide consultation on discharge or admission to the hospital or other facilities. Serve as liaison between hospital and staff regarding resident’s progress. Responsible for maintaining and following all appropriate standards set forth by funders. Attend and participate in monthly meetings as determined by the Director of Nursing. Requirements Education & Experience Licensed Registered Nurse in the State of Missouri. Must maintain Registered Nursing license through continuing education required by the State of Missouri. Knowledge of various types of disabilities. Knowledge of Standards of Care required by funding sources and accrediting bodies. Three years of current nursing experience in both home care and hospital environment. Community health experience preferred. Experience in the field of developmental disabilities preferred. Required Skills/Abilities & Qualifications Ability to read, analyze and interpret common scientific and technical journals, and legal documents.: Ability to calculate figures and amounts such as discounts, proportions, percentages, and volume. Ability to apply concepts of basic algebra and geometry. Excellent verbal and written communication. Ability to prioritize tasks and function well in a high-paced (at times stressful) environment. Valid driver’s license and auto insurance (use of personal vehicle required at times). Acceptable background checks. Physical Requirements Able to stand, walk, and sit four (4) hours at a time. Repetitive bending and reaching up to two (2) hours a day. Ability to lift and carry at least 50 pounds, including having the ability to transfer clients when needed. Ability to access all sites.
