A new opportunity has arisen within the Medicine Care Group at Sheffield Children’s for a Pathway Co-ordinator. You will play a vital part in supporting the smooth delivery of care pathways across a range of specialties, including Gastroenterology, Respiratory, Neurology, Oncology, and more. Working with clinical and management teams, you’ll ensure patients receive efficient, and appropriate care in line with the national 18-week Referral to Treatment (RTT) target.
What We’re Looking For: We’re looking for a compassionate, detail-oriented, and proactive person with:
Knowledge of hospital systems (e.g., PAS) and waiting list policies
IT proficiency (formal training to diploma level or equivalent)
Strong data analysis skills to support performance and service improvements
Understanding of outpatient and inpatient waiting time targets
A commitment to Trust values, inclusion, and continuous improvement

Reporting into the Service Manager, your responsibilities will include:
Track patients through their 18-week RTT pathway
Identify and escalate pathway blockages or delays
Work collaboratively to redesign care pathways for smoother patient journeys
Validate and analyse data to support care delivery
Communicate with a range of clinical specialties and care group teams to ensure service efficiency
