Job Description:
Accountabilities:
General
- Meet with participants via in person and/or video conferencing, travel to two (2) Community Services locations in Kelowna and Westbank; may engage with participants in public settings or in homes, as appropriate.
- Along with other local team members, such as a spiritual care representative, team leader and other supporting PoH volunteers will journey with participants to reach their goals and build hope for the future.
- Support ministry unit integration and encourage the delivery of spiritual care services by each ministry unit team.
- Work with ministry units to produce and promote opportunities for corps members and volunteers to get involved with supporting families.
Ministry Unit Coordination
- Serve as a caseworker for Pathway of Hope for the Community Services locations in Kelowna and Westbank;
- Work with each Community Services office Pathway of Hope team including Officers, employees, congregation members, volunteers, and community partners, to work with participants from a holistic perspective.
- Connect with each location (via phone, video conference, or in-person) to coordinate community linkages, referral and enrollment processes, and any reporting requirements.
- Facilitate ministry unit access to the services of the THQ/DHQ Pathway of Hope team including case consultation and technical assistance, training and development, evaluation, and reporting.
- Ensure program policies and procedures are followed to maintain integrity, accountability, and program outcome measurements.
- Attend and participate in training and meetings as required by the Ministry Units, Divisional and Territorial Headquarters.
- Ensure consistent communication by participating in regular meetings and conference calls with Pathway of Hope teams, divisional personnel, and the THQ Pathway of Hope team.
- Connect regularly with the Community Services Director for support as required and to provide up-to-date and accurate progress reports.
- Connect with the DHQ Divisional PoH Coordinator virtually at least monthly to discuss participant progress, team functioning; to collaborate, promote and organize training opportunities.
Case Management Services
- Perform all aspects of the case management process, initial and ongoing for participants files using the Pathway of Hope approach (selection, intake, assessment, goal planning, action, transition, and follow-up.)
- Perform traditional casework in accordance with program directives for non-POH participants.
- Work with the Pathway of Hope participant and ministry unit teams ongoing to coordinate goal setting and action plans and schedule regular meetings to review progress and engage in further planning.
- Ensure confidentiality is maintained in accordance with The Salvation Army policy and applicable legislation.
- Provide information and referral services as needed for participants and may also include individuals not eligible for support through the Pathway of Hope program.
- Ensure confidentiality is maintained in accordance with Operating Policies and applicable legislation.
- Conduct/arrange in person visits as needed.
- Engage and build rapport with the target population.
- Assist participants in making connections and accessing community resources offer/arrange accompaniment services as appropriate.
- Network with other service providers and maintain working relationships with community agencies to provide comprehensive services for participants.
- Work closely with other local ministry unit personnel to form an interdisciplinary team to work with families from a holistic perspective.
Administration
- Effectively maintain records to accurately reflect services rendered using all appropriate forms, including detailed accurate case notes for every interaction.
- Ensure accurate and timely collection and maintenance of data, submit monthly summary of service statistics to the PoH Coordinator, and assist in semi-annual evaluation/outcome measurements.
5.Community Networking and Partnerships
- Network with other service providers and maintain relationships with community agencies to provide comprehensive services for participants
- Attend agency and community meetings as needed
Perform other duties as assigned.
Critical Relationship Management:
- Internal relationships:
- Supervisor, Corps Officer’s, volunteers, staff, corps members, DHQ Divisional PoH Coordinator and Regional PoH Coordinator.
- External relationships:
- Participants and their families, government agencies and local community support services
MANAGERIAL/Technical Leadership RESPONSIBILITY:
- This position reports directly to the Community Services Director.
- Represent The Salvation Army at agency and community meetings as directed.
- Attend regional Pathway of Hope meetings monthly (in-person or via telephone/video conference).
- This role works independently (within a team) and does not manage internal staff.
- The role acts as a coordinator of work cross-departmentally to ensure comprehensive services are available to participants.
