PATIENT ACCESS CARE ASSOCIATE JOB SUMMARY: Here at Bridgepoint, the Patient Access Care Coordinator Associate is responsible for patient facing registration and associated tasks and case management associated task that include but is not limited to: interviewing patients and or authorized patient representative upon admission, obtaining information necessary for registration, billing and admission, Insurance related tasks that includes verification and collection of associated paperwork; activate registrations, report admissions & transfers, maintain a daily Utilization Review report that reflects current DRG’s and Medicare days for patients obtained from HIM and admissions. Assists case managers and social workers with copying, scanning, and faxing PHI for referrals. Maintains HLOC file for LTAC and SNF, Maintains weekly UR log for reviews and authorization. Maintain documentation in HCS to support admission related authorization and billing tracking and claims processing. Supports case management staff with discharge planning and maintains office supplies, and equipment. Maintains departmental weekly, monthly, and annual reportable data. Case Management Assistant, Social Worker Assistant, Case Manager Assistant, Assistant Case Manager, Patient Access Qualifications Education: High School Education or equivalent Licenses/Certification: Experience: Knowledge of regulatory standards and compliance requirements and hospital policies and procedures as related to discharge planning. Working knowledge and ability to apply professional standards of practice in job situations. Strong organizational and analytical skills. Working knowledge of personal computer and software applications used in job functions (Order entry, data input etc.) Health care experience in admissions, patient registration, business office, a clinical area or a closely related field desired.
