Summary The Patient Care Advocate is responsible for supporting the CMG Medicare population with their medical appointments and any questions. They perform clerical duties using specific knowledge of medical terminology and clinical procedures. Duties and Responsibilities 1. Monitors the referral process to ensure patient completed specialist visit and notes are received. 2. Receives and makes patient calls to schedule/re-schedule appointments and provides instructions from primary clinician. 3. Participates in daily huddles with the clinical staff to review Medicare patients to be seen the following day.    4. Makes follow-up appointments with PCP and in-house specialists. 5. Schedules and coordinates Transportation. 6. Manages cellphone, answers phone calls, takes and leaves messages. 7. Medical Records and Referrals Management. 8. Reviews any referrals with the MCR appointment confirmed status. 9. Review Actions and Telephone encounters. 10. Contacts hospitalized patients. 11. Requests dental appointments. 12. Works on HEDIS reports. 13. Verifies and update patients’ insurance information, addresses and telephone numbers to ensure proper billing and accuracy. 14. Confirms patients’ appointments. 15. Assist patients with their monthly OTC benefit ordering. 16. Performs other duties as assigned.
