Overview The Patient Care Coordinator will support daily operations of a Hospice Care in the home care and facility setting. This includes daily administrative functions, ensuring great patient and family experience, managing CNA schedules and operations for providers, managing the plan of care needs for the CNA and communicating changes to the patients/families so all are informed on a timely basis of any changes. Location: Washington, DC Hours: Monday- Friday: 7:30am-4:30pm COVID-19 vaccine required to be completed upon start Responsibilities The Patient Care Coordinator is included in daily administrative functions, ensuring great patient and family experiences while managing CNA schedules and operations for providers. The Patient Care Coordinator will work with the clinical team and executive leadership to ensure high quality of daily operations, supplies and equipment, and overall support for the office team. Qualifications Experience RequirementsMust have 2 years of practice at comparable level in a healthcare setting, with some elder health interest and experience preferred. Seeking person with high energy and strong work ethic, with attention to detail, who enjoys working with an interdisciplinary team. Education Requirements Must have a high school degree but an Associate’s Degree is desired Driver's License Must have a driver's license in good standing and access to a vehicle during working hours.
