Overview The Patient Care Coordinator I (Bilingual-Spanish) will act as a liaison to inpatient, outpatient, and international patients and families referred to the Nursing Care Managers to arrange appropriate Kennedy Krieger Institute services related to the patient’s special needs. This position requires a high degree of customer service, strong relationship building skills, and cultural sensitivity to manage the needs of clinical staff, patients, and their families. The incumbent will function as a team member with Nursing Care Managers. Responsibilities 1. Provides intake coordination for referrals specified by the Nursing Care Manager. Coordinates and disseminates patient information for review by physicians and Nursing Care Managers. 2. Obtains and documents patient clinical, demographic, and insurance information in designated Kennedy Krieger Institute forms and databases. 3. Researches and documents previous appointments and/or requests for services by internal and/or external referral sources. 4. Communicates with appropriate staff for approval or appropriateness of referral. Follows up with staff and completes all documentation. Finalizes the intake by communicating disposition to the family and/or referral source. 5. Verifies patient is correctly scheduled and insurance clearance is obtained prior to the appointment date. Enters information in designated Kennedy Krieger Institute electronic medical records as needed. 6. Schedules appointments with the Care Center and/or Clinic Coordinators based on Clinic Policy. Notifies appropriate staff members of any critical issues/information regarding appointments, insurance, referrals, Letters of Guarantee, etc., as needed. 7. Serves as a resource to patients and families to explain appointment locations, appointment schedule, and services provided at the Institute. Ensures that the patient’s transition between services goes smoothly, including copying and distributing medical records prior to the visit, and participates in follow-up scheduling as directed. 8. Creates and maintains databases for designated inpatient and outpatient referrals, including appointments, surgery dates, and follow-up appointments as requested. 9. Maintains an ongoing relationship with the Kennedy Krieger Referral Liaison and attends outreach meetings as needed. 10. Produces regular reports of patients served for review by the Director, Nursing Care Management and Kennedy Krieger Institute Administration. Qualifications Bilingual in English and Spanish with a proficiency level that meets the Institute’s minimum standards. Able to speak Spanish fluently with ease and confidence in a variety of topics related to work, home, leisure activities, as well as events of current, public or personal interest or individual relevance. Able to convey intended message with accuracy, clarity and precision without misrepresentation or confusion while being readily understood by native speakers. Able to provide written translation between English and Spanish with a proficiency level that meets the Institute’s minimum standards, ensuring the intended meaning is accurately and clearly communicated. EDUCATION: · High School Diploma or GED required. · Bachelor’s degree in human services or health care administration field preferred. EXPERIENCE: Minimum three (3) years of experience coordinating patient services in a medical setting with children and families, preferably those with special needs, required. Minimum pay range USD $50,148.00/Yr. Maximum pay range USD $85,998.00/Yr.
