About the Company Our partner is a virtual-first specialty clinic providing compassionate, evidence-based care for patients with rare and complex chronic conditions like Ehlers-Danlos Syndrome (EDS), POTS, MCAS, and related disorders. By using advanced digital health tools, they break down barriers to care and support patients who are often underserved and misdiagnosed. Their mission is to expand access to specialized care and improve patients’ quality of life. The team operates in a fully remote, fast-paced environment that values empathy, initiative, and operational excellence. Responsibilities: Patient Support & Communication: 50% Act as the first line of contact for patient inquiries across platforms—EHR, secure chat, and phone—handling scheduling, prescription coordination, and follow-ups with clarity and empathy. Care Coordination & Navigation: 40% Work closely with our clinical team and external providers to coordinate diagnostic testing, imaging, referrals, and prior authorizations. Track patient progress across the care journey and ensure continuity of care. Operational Workflow Management: 10% Maintain HIPAA-compliant documentation, support onboarding for new patients, and flag care gaps and systemic or financial barriers to care. Participate in team meetings and continuous process improvement efforts. Use and/or create automations to create leverage; currently manual tasks - responding to humans and chart preparation Respond to inbound messages and respond to patients Work with 2 existing patient coordinators Medical Scribe: Serve as a Medical Scribe, accurately documenting patient encounters, physician notes, and other relevant information in the electronic medical record (EMR). Ensure timely and precise medical documentation to support high-quality patient care. Tools: Elation Health (EHR) Slack Google Workspace (Gmail, Sheets, Drive) Zoom Spruce Health (for patient messaging & VOIP) Notion (internal knowledge base) Must haves: Prior experience in care coordination, case management, or virtual clinical support (1-2 years of US healthcare experience) 1 year of experience as a Medical Scribe in a clinical or telehealth setting.
