What you'll do day-to-day: Reach out to your assigned patients on a structured schedule — some weekly, some monthly — based on their condition and risk level Review remote monitoring dashboards and proactively contact patients when data is trending in the wrong direction Help patients navigate real-world barriers: late refills, insurance questions, transportation, specialist coordination, copay assistance Document patient interactions using AI-assisted tools (less typing, more connecting) Escalate clinical concerns to an on-team pharmacist using clear protocols Assist to enroll newly eligible patients into the program Why this work matters Chronic disease is won or lost between appointments. The details between appointments matter just as much as the information shared during the appointment. Catching when a patient's blood pressure is quietly trending up, whether they understand their new medication, if a refill is stilling uncollected due to cost or transportation. We keep our patients engaged in their own care.
