Position Summary The Patient Care Coordinator serves as a key liaison between patients, healthcare providers, and administrative staff. This role ensures that patients receive timely, compassionate, and well-organized care by managing appointments, coordinating services, handling insurance authorizations, and providing clear communication throughout the care process. The ideal candidate is empathetic, detail-oriented, and highly organized with strong interpersonal and customer service skills. The successful candidate will be responsible for the following: Key Responsibilities: Serve as the primary point of contact for patients regarding appointments, follow-ups, and care plans Schedule, confirm, and coordinate patient appointments, tests, and procedures Ensure patients understand their treatment plans, medications, and provider instructions Communicate with medical staff, specialists, and other healthcare professionals to coordinate comprehensive care Assist with insurance verification, pre-authorizations, and referrals Maintain accurate and up-to-date patient records in the EMR/EHR system Monitor patient progress and follow up as needed to ensure continuity of care Respond to patient inquiries, complaints, and concerns with professionalism and empathy Support quality assurance initiatives and patient satisfaction goals Educate patients on clinic policies, forms, and documentation Qualifications: High school diploma or GED required; Associate’s or Bachelor’s degree in healthcare, business, or a related field preferred 2+ years of experience in a healthcare or clinical setting, preferably in a patient-facing role Familiarity with medical terminology and health insurance processes Experience with electronic medical records EMR: Athena Preferred Excellent communication and interpersonal skills Strong organizational and time-management abilities Compassionate, patient-focused attitude Bilingual skills are a plus Preferred Skills: Knowledge of HIPAA and healthcare regulations Background in nursing, medical assisting, or case management is a plus Ability to multitask in a fast-paced clinical environment Proficiency with Microsoft Office and scheduling software
