Values: Respect, Commitment, Diversity, Competence, Education, Research, and Cost Effectiveness Job Summary This role is designed to welcome, onboard, and support new patients entering a busy primary care setting within a Federally Qualified Health Center. The Clinical Care Coordinator – New Patient Ambassador works closely with nurses, providers, and the broader care team to ensure each new patient experiences a smooth, informed, and coordinated introduction to care. Requirements Qualifications High School Diploma or Equivalent Certification in Medical Assistant training or enrollment in nursing school, physician assistant school, or completion of a bachelor’s degree with health science training Minimum 2 years of relevant experience preferred Job Duties: Clinical Support Duties (25%-50%) Obtain vital signs, perform point-of-care testing, EKGs, and screenings. Assist nurses and providers with clinical tasks including vaccine and medication administration, procedures, and pre-visit planning. Prepare and maintain exam rooms. Greet and room patients, update EHR information, and assist with scheduling follow-up visits. Communicate effectively with patients regarding wait times and care updates. Provide excellent customer service. Maintain infection control standards. Transport patients via wheelchair as needed. New Patient Care Coordination Duties (50-75%) Manage and respond to incoming new patient inquiries. Coordinate and schedule all new patient appointments, including establish-care and urgent visits. Verify previous care relationships and align patients with appropriate care teams. Request, obtain, and organize outside medical records. Collaborate with internal departments and external partners. Track and document all new patient coordination activities in the EHR. Assumes other duties as requested. Benefits We value your expertise and commitment.
