Job title: Patient Coordinator
Hours: 30 - 37 (various shifts between Monday to Friday 6.45am-8.00pm and Saturday 8.30am-5pm)
Holidays: 28 days pro rata plus bank holidays

THE FAKENHAM MEDICAL PRACTICE
A vacancy has arisen for an enthusiastic patient coordinator, to join our friendly, busy GP practice based in Fakenham, North Norfolk.
We are looking for a dynamic, reliable, hardworking and flexible person to join our team. This is a demanding role, assisting our 15000 patients, so you will need to be confident, highly organised, have excellent communication skills and good at multi-tasking.
Our patient Coordinators are the first point of contact for our patients, this can be either on the telephone, face to face or online, they assist our patients in receiving the right care from the most appropriate clinician. The successful candidate will be able to demonstrate sensitivity, respect, tact working in a confidential environment. You will be joining a friendly supportive team working alongside a clinician.
We work from modern, purpose-built premises. The partners and staff have a shared belief in the delivery of high-quality healthcare. Patients’ interests are kept very much to the fore and there is a strong commitment to develop patient services. To promote a multi-disciplinary team approach, we share our premises with other providers including a pharmacy, adult social services and local community services e.g. physiotherapy and midwifery.
· Receive patients and visitors on our front desk reception.
· Be able to cover all reception positions as necessary, front desk, phones, website questions and requests, emails and clinical tasks.
· Respond efficiently and courteously to patients queries and requests.
· At the start of the day, if you arrive first, make the necessary preparations to receive patients and visitors.
· When last to leave at the end of the day, ensure the internal lights are off and the building is secure.
· Liaise with hospitals and other NHS organisations as required.
· As directed by our triage clinician book the appropriate appointment or telephone consultation for the patient.
· To answer the emergency phone.
· Maintain effective communication within the practice, with patients, visitors and colleagues.
· To undertake regular training and development, to keep personal development plan up to date and to attend staff meetings.
· To undertake any other activities that may from time to time be reasonably requested by the Patient coordinator lead, Operations Manager, Practice Manager or Partners.
