We are looking for Patient Education Specialist – Behavioral Health to join our clinics! The Patient Education Specialist - Behavioral Health works with the behavioral health team to help implement behavioral health change plans, coordinate care, and connects patients to appropriate community resources and providers. The Patient Education Specialist also works directly with patients receiving medications for opioid use disorder to assess social determinants of health and develop plans for patients to address areas of need, including connection with resources. The Patient Education Specialist functions as a health educator and as part of the clinical team, working with the provider, medical assistant, other clinic staff, and the family to provide brief coaching, discreet skill building, and coordination in accessing supports to address identified needs. Plus other duties as assigned. This position is grant funded and contingent. Qualifications Bachelor’s degree from an accredited college or university preferably in behavioral health, psychology, social work, or child development, or a closely related field; or equivalent experience We encourage anyone with a relevant combination of education and experience to apply.
Responsibilities
