Patient Flow Coordinator - SJCCC
6 Month Full-Time Contract (with possibility of extension)
Hourly rate: $31.69-$39.62
Start date: as soon as possible once all pre-employment conditions are met
The Patient Flow Coordinator manages application and pre-admission processes by reviewing applications to determine patients’ eligibility for placement in rehabilitative care programs and liaising extensively with referring agencies to define specific patient vulnerabilities, abilities, care requirements and discharge goals. They match patients’ rehabilitation goals with the program offerings of the organization to ensure appropriate fit to enhance and maximize functional abilities. They advise on admission needs of patients to ensure smooth transitions. The Patient Flow Coordinator works as part of the interprofessional team and acts as a consultant to other disciplines.

QUALIFICATIONS:

  • Regulated Health Professional registered with the applicable professional college
  • Two to three years of experience in rehabilitative care environment
  • Knowledge of and experience in determining patient eligibility for rehabilitative care programs in collaboration with referring agencies
  • Excellent communications skills to interact with the interprofessional team and system partners
  • Strong interpersonal, organizational, and people management skills
  • Preference given to candidates with knowledge of SJCCC and its programs
  • COVID-19 Vaccination mandatory (2 Doses)
