We are seeking a Patient Navigator to support a new outreach initiative.This role plays a vital part in connecting patients to ongoing preventive, primary, and specialty care, while also leading efforts to enhance community engagement and implement technology-driven health initiatives.Position SummaryThe Patient Navigator will help increase access to care, strengthen community relationships, and support key health outreach programs, including the implementation of the Unite Us platform and cancer screening coordination efforts. This is a temporary, full-time position with funding secured for 12 months.Key Responsibilities Connect patients from urgent care to preventive and primary care services across BMS. Provide navigation support for at least 60 new patients per month, including adolescents. Track and document patient referrals, ensuring follow-up completion and outcomes reporting. Implement and manage the Unite Us platform to improve SDOH (social determinants of health) referrals. Support outreach and community engagement activities related to cancer screening programs. Collaborate with internal teams (including Operations, Community Health, and Behavioral Health) to align outreach strategies. Assist with data collection, program tracking, and reporting for grant deliverables. Requirements Qualifications Bachelor’s degree in Public Health, Health Administration, Communications, or related field required. At least 2 years of experience in community outreach, patient navigation, or care coordination. Strong technology and media skills — must be comfortable using digital tools and outreach platforms. Excellent communication, organization, and problem-solving skills. Experience with social service referrals, SDOH frameworks, or healthcare technology platforms a plus.
