Description Comprehensive Blood and Cancer Center is looking for a Patient Navigator to join our Navigation team. Patient Navigators offer support with educational, practical, emotional, or logistical issues that may arise with a diagnosis of cancer. EDUCATION & EXPERIENCE: Any combination equivalent to the experience and education that are required by Federal, State, and local guidelines. The minimum requirements for experience and education are as follows: Diploma or GED for High School Minimum 3 years of medical scheduling experience Medical Assistant Certification - desired RESPONSIBILITIES: Review educational material with the patient as directed. Provide consultation planning services with the patient as requested (assisting clients in making a list of questions for medical oncology consultations). Assist patients and referring providers with scheduling initial consultations. Communicate with oncologists regarding new patient referrals Ensure that pertinent medical records are available at the first appointment. Work with auxiliary departments to ensure patient appointments are coordinated promptly and conveniently. Keep clear documentation of all communication. Link patients with appropriate appointments Medical oncology Radiation oncology Surgical intervention Chemotherapy education Radiation education Social Services Financial liaison Clinical research (if available) Genetic screening Ensure patient has information regarding resources available to them Transportation Co-pay assistance Drug assistance programs CBCC foundation programs Prepare routine departmental correspondence to referring agencies and primary care providers. Support patients' treatment decisions, no matter what they might be.
Responsibilities
The Patient Navigator provides educational, emotional, and logistical support to cancer patients while coordinating consultations and treatment schedules. They act as a liaison between patients, oncologists, and auxiliary departments to ensure seamless care delivery.
