Responsibilities Under the Direct Supervision of the VP of Physician Services, the Patient Navigator will work in collaboration and continuous partnership with patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources to connect patients to relevant community resources, with the goal of enhancing patient health, well-being, decreasing patient readmissions and increasing patient satisfaction. Qualifications MINIMUM EDUCATION: High School Graduate or GED Equivalent; relevant experience in lieu of High School Diploma/GED education will be considered. MINIMUM WORK EXPERIENCE: 3 years previous customer service experience. REQUIRED LICENSES/CERTIFICATIONS: None REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES: Previous experience in a customer support role. Strong phone and verbal communication skills along with active listening. Customer focus and adaptability to different personality types. Ability to multi-task, priorities, and manage time effectively. Ability to establish and maintain strong working relationships with staff and management; excellent organizational skills with emphasis on detail and ability to effectively communicate both verbally and in writing. Detailed oriented, team player, and an independent thinker.
