UPMC is looking for a Patient Navigator to join their team! Are you someone who is client-centered, strengths-based and empathetic? If so, apply today! Hours: Monday-Friday 8am-4:30pm with 1 evening required until 6:30pm. Monday or Tuesday nights would be the required late evening due to clinic schedule and support group schedules. This is a great opportunity to help in your community! Primary Duties – Medical Case Management: Maintain a caseload of approximately 40 medical case management clients. Perform and follow-up according to Part B Ryan White medical case management standards for these clients. Link clients to community resources and departmental resource such as mental health care, substance abuse services, housing, to name a few. Ensure all clients applying for financial assistance meet eligibility and financial requirements Work one-on-one and in a group setting with clients to become self-advocates along their HIV diagnosis and treatment journeys. Offer a safe space for clients to express their perceived strengths, barriers to treatment, and explore individualized strategies for success. Provide medication reminders and empowerment. Secondary Duties: Provide rapid HIV testing on-site at REACCH and off-site in the community. Participate in community outreach & engagement events. Provide follow-up with newly diagnosed clients at UPMC hospitals, off-site outpatient offices, and on-site at the REACCH Program, as needed. Other duties as assigned. · Bachelor’s degree in social work, human services, health care or a related field required. · An ability to demonstrate one’s commitment to social justice strongly preferred. · Experience providing case management and knowledgeable about community resources in Dauphin County preferred. · Demonstrated experience exhibiting exceptional organizational, assessment, teamwork, and critical thinking skills. · Demonstrated ability to motivate and work with people with diverse backgrounds. · Ability to navigate resources and work independently serving case management clients. · Experience in the HIV/AIDS field preferred. · Bilingual in English and Spanish is preferred. · Understanding of and the ability to apply motivational interviewing, solution-focused, and strengths-based client-focused techniques preferred.
