The Floating Hospital welcomes individuals of all ages, from infants, school age children (5+), youth and adults. TFH is a Federally Qualified Health Center and the largest provider to homeless families living in family shelters and domestic violence safe houses throughout all five boroughs. The Position: We are seeking a Patient Navigator to join our Patient Navigation team at our Long Island City (Queens) facility. The ideal candidate will have an outgoing personality, aide in creating a welcoming environment, and provide an overall positive impression for patients, their families, and visitors to our facility. Under the direct supervision of the Patient Navigation Manager, the primary responsibilities include: Greet patients, families, and visitors entering the facility. Inform the front desk that patients have arrived for their visit. Answer questions appropriately and professionally, executing the appropriate action when necessary. Provide patients with the necessary paperwork that needs to be completed. Collaborate with the front desk to ensure all patient needs and questions are satisfied. Escort patients to and from exam rooms. Ensure that social distancing guidelines are adhered to. Other duties as assigned. Requirements Qualifications and Experience: Education: Candidates must possess a high school diploma or equivalent Experience: Strong customer service experience, possess excellent communication skills, be organized, and be able to function in a fast-paced work environment. Language: Fluency in English and Spanish is a requirement. Responsibilities
