Sherwood Forest Hospitals have a unique opportunity to join a passionate, professional, and dedicated team that is focused on creating a positive journey for all patients seen within our service.
You will be the main point of contact for patients throughout their pathway of care. The post holder will work under the direction of their Senior Patient Pathway Coordinator and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly and in a timely manner.
From day one, you will be communicating with individuals both internally and externally at all levels; you will need to do so with confidence and integrity while embodying our culture and our care values.
It is imperative that the successful candidate can always maintain complete confidentiality on all people-related matters.
The work is fast-paced and challenging but very rewarding. Your experienced colleagues will provide you with a robust training program that will help you build the skills, knowledge, and confidence needed to become a successful Patient Pathway Coordinator.
