PATIENT PATHWAY CO-ORDINATOR

Location: 131 Psalter Lane, Sheffield, S11 8UX.
Salary: £28,000.00 per annum, with excellent benefits.
Contract: Permanent, Full time, 40 hours per week, Monday to Friday, with a paid lunch break.

KNOWLEDGE AND SKILLS:

Responsibilities
  • Working collaboratively with administrative, clinical, and management teams, maintaining consistent communication with administrative staff to support the progression of patient pathways and ensuring the accuracy of waiting lists at all stages.
  • Ensuring all patients are booked within the 18-week referral-to-treatment (RTT) target and responsible for the daily validation of pathways in line with agreed practices and standards.
  • Ensuring that no PLCV procedures are carried out without the appropriate authorisation from the ICB.
  • Managing anaesthetist cover for all thatre lists across multiple hospital sites, coordinating cover in advance with a proactive mindset for uninterrupted service delivery.
  • Monitoring and managing NHS Trust contract work, updating trackers, producing reports and overseeing patient pathways to ensure contractual obligations and delivery of high-quality care.
