Description The Patient Service Representative (PSR) – Medical Records is a hybrid role responsible for managing patient medical records while also supporting front-office patient service functions. This position requires the ability to balance administrative medical records duties with direct patient interaction, including answering phones and assisting with general PSR responsibilities. The ideal candidate is organized, detail-oriented, and comfortable working in a fast-paced healthcare environment. Medical Records Responsibilities: Maintain, organize, and update electronic and/or paper medical records Ensure medical records are complete, accurate, and compliant with organizational standards Process and track requests for medical records in accordance with HIPAA regulations Verify patient identity and authorization prior to releasing medical records Scan, index, and upload documentation into the EHR system Assist with audits, quality checks, and record reconciliation Patient Service Representative (PSR) Responsibilities: Answer and route incoming phone calls in a professional and courteous manner Schedule appointments, verify demographics, and update patient information Respond to patient inquiries and provide general office information Coordinate communication between patients, providers, and staff Support front-desk operations as needed, including check-in/check-out coverage General Responsibilities: Manage competing priorities between medical records and PSR duties Maintain strict confidentiality and adhere to HIPAA and privacy policies Collaborate effectively with clinical and administrative staff Perform other duties as assigned to support clinic operations Requirements Qualifications High school diploma or equivalent required; healthcare or medical office training preferred Prior experience in medical records, patient services, or healthcare administration preferred Knowledge of HIPAA and medical records release requirements Experience with EHR systems and multi-line phone systems Strong organizational skills with the ability to multitask and shift priorities Excellent customer service and communication skills Ability to work independently and as part of a team
