The PSC is responsible for delivering excellent customer service and supporting key initiatives related to patient adherence, discharge planning, and retention. The PSC also participates in special programs, handling data analysis, reporting, and specific therapy tasks, and collaborates with stakeholders to ensure the success of program goals and initiatives. Reporting to: Departmental Supervisor or Manager Job Type: Full-time, Mon-Fri 8:30am-5:00pm Location: New Providence, NJ (hybrid 2 days in the office and 3 days remote) Requirements High school diploma or equivalent. Bachelor’s degree in healthcare field or business is preferred Strong data analysis and organizational skills Strong attention to detail and accuracy Experience with patient admissions in a similar referral source-based practice setting and customer service in an inbound/outbound customer service center is strongly preferred Customer service experience in a Pharmacy Specialty is strongly preferred, but previous customer service experience in the healthcare services business is also acceptable Previous experience working with infusion therapy patients is highly desirable Job Responsibilities, included but not limited to: Conduct telephonic outreach and engagement, including but limited to: problem solving, patient education, adherence support, and retention Medication and supply delivery scheduling Collaborating with nursing department and outside agencies for prescription or supply needs Advocating and raising awareness on behalf of patients and special program initiatives Analyze reports and report outcomes data for special program assignments Utilize good time management skills to ensure all facets of patient engagement activities and program requirements are completed in a timely manner Participate in retention and discharge committee meetings as assigned Conduct proactive, concurrent, and retrospective audits to ensure all charts and tickets are complete, compliant, and present Prepare, package and label medications for deliveries Track equipment Manage inventory and order supplies as needed Maintain a clean and organized work environment Adhere to all pharmacy regulations and guidelines Other additional job duties Physical Demands The physical requirements listed here indicate what an employee must meet to effectively perform this role's essential functions.
