The Position 患者全程管理 1.在生态圈内绘制患者就医流转图，从而更好的推动全程管理在医院内/间的落地。 2.协助生态圈内医院及相关科室推进和打造会诊通道，协助上下层医院间衔接流转，助力绿色转诊通道的建立，最终实现患者获益。 3.管理生态圈内患者全程管理项目的各项落地实施，并确保其合规性。支付方案与准入团队合作，协助整合罗氏相关支付方案，传递完整支付解决方案信息。疾病认知（患者教育) 与第三方机构协作，针对相关疾病领域区域现状及整体策略，协助开展患者教育，提升疾病认知，促进患者获益。工作年限 •5年以上医药行业工作经验，有管理经验者优先 •有跨职能、或者治疗领域经验优先教育程度/所获资格证书 •医学或药学相关本科及以上学历 Who we are A healthier future drives us to innovate. Responsibilities

The Patient Solution Manager is responsible for mapping patient care pathways within the healthcare ecosystem and facilitating the implementation of patient management projects. They will also collaborate with hospitals and departments to establish consultation pathways and ensure compliance.
