JOB DESCRIPTION OVERVIEW Under the supervision of the Director of Public Programs, the Prevention Care Coordination (“PCC”) Program Manager provides oversight and management of program operations; supervises and supports program staff; collaborates with Men’s Health Foundation leadership to develop and implement strategic goals for the program and represents the PCC Program in the community and within the MHF organization.   ESSENTIAL FUNCTIONS AND RESPONSIBILITIES  * Provide direct, on-site management of the PCC Program; ensure the program is implemented according to MHF policies, procedures, and protocols. * Build and monitor evolving PCC operations to ensure that the program meets all grant-required goals and objectives, addresses new barriers to care, and implements continuous quality improvement strategies to strengthen program effectiveness and client outcomes. * Ensure the site is clean and well maintained. * Oversee day-to-day PrEP program operations to ensure efficient and client-centered service delivery. * Develop and update workflows and SOPs to improve program effectiveness. * Provide leadership and supervision of PCC program staff, offering feedback and mentorship, and ensuring all staff members deliver consistent, high-quality services for patients, clients, and participants. * Build and maintain PCC operations and ensure that the program meets all grant-required goals and objectives. * Proactively identify and address barriers to program success in collaboration with MHF leadership. Identify and reduce barriers to PrEP access (cost, stigma, transportation, language, etc.). * Track program metrics (enrollment, retention, adherence, demographics, outcomes). * Ensure culturally responsive and trauma-informed services. * Support linkage to insurance, medication assistance programs, and supportive services. * Prepare and submit grant and funder reports on time. * Conduct regular check-ins to address current needs, provide performance feedback, and identify professional development opportunities for team members. * Work with the MHF Senior Leadership Team to develop and implement a long-term strategy to expand the PCC program to meet the evolving needs of the target population. * Maintain and routinely audit records and tracking spreadsheets to ensure accuracy, completeness, and timely updates. * Develop data reports that support data-driven decision-making, help identify program needs and trends, identify gaps, and guide program improvements. * Plan and coordinate PrEP-related outreach and education events by managing timelines, securing locations, organizing supplies and materials, coordinating staffing, and supporting day-of-event activities to ensure effective community engagement and service delivery. * Represent Men’s Health Foundation at professional events, building relationships with community partners to expand awareness of the PCC Program. * Collaborate with MHF staff from other programs to ensure the PCC program functions as an integral part of MHF’s suite of healthcare services. * Monitor quality of services and ensure compliance with contract terms as well as all state, county, and federal standards and regulations. * Provide reports as required or requested on contract compliance activities and program outcomes. * Other duties may be assigned as required.   QUALIFICATIONS * Eight combined years of post-secondary education and experience working in the HIV field. * Strong track record of working with LGBT persons living with HIV.  * Excellent communication skills (oral and written) and interpersonal skills for frequent contact with internal customers and external stakeholders. * Ability to work independently with proactive style, taking initiative to identify and address potential issues. * Strong problem solving and analytical skills; systems thinking. * Technology and software savvy; ability to utilize MS Office Suite and Project Management software/programs. * Ability to lead and positively influence a diverse team of individuals with strong connections to the community and with varying levels of professional experience. Excellent communication and organizational skills; strong written and interpersonal skills. * Bilingual/Spanish preferred. * Strong commitment to the Mission and Vision of Men’s Health Foundation. * Demonstrated ability to provide services to people of diverse races, ethnicities, sexual orientations, gender identities, nationalities, economic backgrounds, religions, ages, English-speaking abilities, immigration status, housing status, and physical abilities in a multicultural environment. COMPANY REQUIREMENTS * Must be able to pass a pre-employment drug test, physical, and a background check to include a 7-year criminal, 10-year SSN & employer history reference check.  * Must be able to provide proof of COVID-19 vaccination on the first day of work.   * Excellent interpersonal skills. * Attention to detail. * Must take yearly flu shot or wear flu mask during flu season for patient-facing positions and test for tuberculosis as required by the Centers for Disease Control and Prevention.
