Position Type: Full-time, First Shift No nights, weekends, or holidays – and free parking! -------------------------------------------------------------------------------- Why General Physician, P.C.? * Work-Life Balance: Enjoy a Monday–Friday daytime schedule with no evenings, weekends, or holidays. * Comprehensive Benefits: Medical, dental, vision, and more. * Generous PTO: Rest and recharge with paid time off. * 401(K) with 3% Employer Contribution: Build your financial future. * Employee Discounts: Including exclusive Verizon Wireless savings. -------------------------------------------------------------------------------- What you will be doing: * Extract, compile, and analyze data from EHRs, dashboards, and payer portals. * Track provider and clinic performance on key quality metrics such as HEDIS, MIPS, PCMH milestones, preventive screenings, and chronic disease management. * Prepare and distribute regular performance reports to providers and clinic managers. * Highlight successes and opportunities for improvement using data visualization and summary reports. * Ensure data accuracy and timely updates in population health tools and tracking systems. * Serve as a liaison between the clinical quality team and frontline care teams. * Meet regularly with providers and clinic managers to review progress toward quality targets. * Provide insight into trends, barriers, and variances in performance. * Assist with interpreting data and recommending process improvements to support quality initiatives. * Participate in team huddles, staff meetings, and quality improvement projects. * Support PCMH recognition and compliance by monitoring data elements tied to NCQA standards. * Assist in documentation and reporting required for audits, certifications, and value-based program submissions. * Collaborate cross-functionally with Quality, IT, Population Health, and Clinical Operations teams to align data strategies with care transformation efforts. * Contribute to the development of dashboards, templates, and workflows that streamline reporting and drive outcomes. * Deliver exemplary customer service to support a positive patient experience across the organization. * Perform other duties as assigned by management. -------------------------------------------------------------------------------- Education and experience you need: * Education (Minimum Needed): Associate’s degree in Healthcare Administration, Public Health, Health Information Management, or related field required, Bachelors degree preferred. Relevant experience in lieu of a degree will be considered. * Experience (Minimum Needed): 2 years’ experience in healthcare quality, population health, PCMH operations, or data analytics; at least 1 years’ experience working in a multi-site clinic or health system environment preferred * Specialized Knowledge Needed for Performance of Job: strong knowledge of clinical quality programs such as HEDIS, MIPS, PCMH, or ACO quality frameworks; knowledge of NCQA PCMH standards and healthcare quality reporting * Special Skills: detail oriented, data-driven with strong communication and analytical skills; strong interpersonal and communication skills; must be able to interpret and present clinical data clearly and concisely * Other: Proficient in Microsoft Excel and population health or EHR-based reporting systems (e.g. eClinicalWorks, EPIC, Azcara, Athena, etc.); Familiar with data visualization tools (Power BI, Tableau, etc.) -------------------------------------------------------------------------------- Join General Physician, P.C., and make a meaningful impact on improving clinical quality outcomes and patient care across Western New York!
