The Robert C. Byrd Clinic, on the campus of the West Virginia School of Osteopathic Medicine, is seeking a full-time Pediatric Patient Navigator.
POSITION QUALIFICATIONS:
- Bachelor’s degree in health sciences, social sciences, public health, or a related field required. Equivalent education, training, and experience will be considered.
- Strong understanding of social determinants of health and their impact on maternal, infant, and child health.
- Demonstrated ability to build trust and rapport with families and collaborate effectively with healthcare providers and community partners.
- Excellent verbal and written communication skills.
- High level of organizational and time management skills to manage multiple cases and priorities.
- Proficiency in using electronic health records and other documentation tools.
EXPERIENCE:
- Minimum of two years of experience in healthcare, care coordination, patient navigation, perinatal services, or social services, with an emphasis on pediatric or maternal health.
- Experience working in a patient-facing role or with vulnerable populations is highly desirable.

JOB REQUIREMENTS:
- Collaborate with the West Virginia Perinatal Partnership to support initiatives aimed at improving perinatal and pediatric care outcomes.
- Implement an integrated, comprehensive healthcare model in the pediatric department to address the needs of pediatric and postpartum populations affected by Substance Use Disorder (SUD) and Opioid Use Disorder (OUD).
- Assist in expanding access to primary care and behavioral health care services - including SUD treatment, Medication-Assisted-Treatment (MAT), maternal mental health, contraception, children’s developmental screenings, and Hepatitis C screening – using the RCBC mobile unit.
- Provide health navigation services to assist patients in obtaining coverage through Medicaid and other health insurance programs.
- Actively participate in developing a sustainability plan to ensure the continuation of services for the target population.
- Collaborate on strategies to expand effective, evidence-based services for the target population.
- Share aggregate data and Performance Improvement and Measurement System (PIMS) measures with the West Virginia Perinatal Partnership for performance reporting on the project.
- Serve as a client advocate, coordinating necessary services to support patient needs.
- Maintain a comprehensive list of available community resources for at-risk families, addressing all ages and insurance types.
- Collaborate with the RCBC multidisciplinary team (social workers, pharmacist) to connect patients with appropriate programs and resources.
- Attend resource fairs to identify and network with community resources for children and caregivers.
- Maintain access to coupon programs for infant formulas, medications, and medical equipment.
- Assist parents of children with special health conditions in completing the “WV Children with Special Healthcare Needs” application and accessing additional resources.
- Work with local support systems to enhance service distribution for patients and families.
- Monitor and provide support for patients facing school truancy, homebound status, and verification of doctor excuses.
- Offer resources and support to grandparents and other caregivers, guiding them through their legal rights and assisting with necessary documentation for patient records.
- Connect families providing foster care for clinic patients with required resources and support.
- Facilitate referrals to community agencies, including WIC, CPS, Mountain Heart, Communities in Schools, and other relevant organizations.
- Educate patients and families about available programs and services through the West Virginia Perinatal Partnership and other community-based organizations.
- Maintain accurate and up-to-date patient records in compliance with organizational and regulatory standards, ensuring strict confidentiality of treatment records.
- Participate in program evaluation and quality improvement efforts to enhance services provided through the West Virginia Perinatal Partnership.
- Attend regular meetings, strategic planning sessions, trainings, and other activities organized by the West Virginia Perinatal Partnership.
- Perform other duties as assigned.
